Patients with a wide variety of chronic or acute health conditions, such as cognitive impairments, injuries from a fall, multiple sclerosis, stroke recovery or swallowing disorders.

How long will I be there?

The in-home occupational therapist, physical therapist or speech-language therapist will develop a therapeutic plan and goals to improve quality of life and well-being for the patient. The therapist will generally come to the home one to three times per week. Once the goals have been met in-conjunction with the patient’s healthcare team, the patient is discharged from care. A typical timeline is a few sessions to weeks therapy.
